The CalAIM Enhanced Care Management (ECM) and Partners for
Independence (PFI) programs provide social and health care
management for people ages 21 or older who are eligible for
Medi-Cal and are Health Plan of San Mateo (HPSM) members.
Both CalAIM ECM and PFI programs are voluntary programs providing
intensive care management to older adults and/or people with
disabilities with medical and psychosocial needs.
