Retail Food Program Forms and Information

Food Health Permits are getting a new look. The new permits will now expire one year after they’re issued—so you don’t have to search for a permit that was issued decades ago. During your annual renewal period, you’ll receive an invoice for the new permit cost. Pay it and we’ll send you a new permit for the next year. As always, businesses operating without a permit will pay three times the standard permit fee and are subject to closure.

The new permits also include information about where to post them, what the permit certifies the owner to do, and are portrait-sized to match our new food placards. We will start issuing new permits July 1, 2016 to new applicants and renewals. Existing businesses can expect to get their new permit after their next inspection after July 1, 2016.

Please call for an appointment to review the file on a food facility that you may be interested in or to talk with the inspector for that area.
